Is this request for On-Campus Housing?

FORM NAME FORM PURPOSE
Request maintenance on anything that is not working properly in or outside of a building.

(E.g. light bulb out, sink is not draining, door handle broken, room is too hot/too cold, chair broken, pest control, etc.)


Request setups and Facilities support for events.

To ensure staff and equipment availability, a work request must be submitted to the Facilities Service Center via the iService Desk at least fourteen calendar days prior to the event.

Request custodial services. (E.g., paper towel/toilet paper dispenser is empty, spill on floor, etc.)

Request “special” custodial support. (E.g. detailed cleaning of an office, out-of-cycle carpet cleaning, etc.)



Request relocation of furniture, equipment, or UTSA Artwork.
